Charlotte Health Initiative Inc. is a nonprofit corporation organized and operated exclusively for charitable purposes, specifically to foster and enhance the health and well-being of indigent patients living with HIV/AIDS and other chronic diseases by providing them with access to quality health care.
Charlotte Health Initiative will provide medical case management services to uninsured and underinsured individuals living with HIV/AIDS and other chronic diseases. We will provide one-on-one intense counseling and case management. We will link patients to medical providers and provide the funding for treatment of their chronic medical condition. This will include doctor’s visits, blood tests, medications, and co-payment assistance. We will assist patients in accessing services that they might need such as specialty care, drug adherence counseling, and Pharmaceutical sponsored patient assistance programs. Charlotte Health Initiative will support programs and studies to identify barriers to obtaining care and retaining patients living with HIV/AIDS in care and will provide solutions to gaps in the treatment of people with chronic diseases and HIV/AIDS in particular.
Despite the Affordable Care Act which now makes it possible for many to secure health insurance coverage, there is still a vast number who are still uninsured or underinsured and so continue to struggle to access medical care and to stay in care. Charlotte Health Initiative will be an advocate and resource to bridge this gap.
The officers and directors administer this program on a volunteer basis. This program has already commenced and consumes 100% of the organization’s time. Initially, the organization’s activities will be advertised through collaboration with other providers in the area, newsletters, brochures, word of mouth, email, and personal contacts; and funded through government grants, and donations from family, friends and the general public. Planning for our program is conducted at 4115 The Plaza Charlotte, NC 28205. Through the successful execution of our program, we further our charitable purposes.

Dr. Tagbo J. Ekwonu is a family medicine physician at Eastowne Family Physicians, PA in Charlotte, North Carolina. Patients in the NoDa, Eastway, Plaza Midwood and other areas of Charlotte depend on Dr. Ekwonu for various medical services, including high blood pressure treatment, diabetes treatment, COPD treatment, high cholesterol treatment, and all kinds of general medical care for the family.
A graduate of the University of Nigeria College of Medicine in Nsukka, Nigeria, Dr. Ekwonu then went on to complete both an internship and a residency at the Bronx-Lebanon Medical Center in Bronx, New York. Dr. Ekwonu is board-certified by the American Board of Family Medicine and holds additional certifications from the American Academy of Addiction Medicine and the American Academy of HIV Medicine.
Dr. Ekwonu is affiliated with the American Academy of Family Physicians, the American Medical Association, the American Academy of HIV Medicine, and the HIV Medical Association. Dr. Ekwonu’s goal is to provide the highest quality care for the entire family. He’s currently welcoming new patients and looks forward to seeing established patients again.
